    I understand that massage therapy provided by Serenity on Demand, LLC. is therapeutic in nature. I understand that all massage services provided by this practice are non-sexual. No sexual conversation or behavior will be tolerated by either the massage therapist or the client at any time. Any inappropriate words, jokes, or suggestions by the client will result in the immediate termination of the massage session. Massage sessions terminated for inappropriate behavior will be charged the full session fee and this practice will be closed to you with no referral.

    I understand that the massage/bodywork I receive is for the purpose of stress reduction and relief from muscular tension, spasm or pain and to increase circulation. If I experience any pain or discomfort, I will immediately inform my massage therapist that the pressure and/or methods can be adjusted to my comfort level.

    Massage therapy in general can provide the benefits of enhanced relaxation, stress reduction, reduced pain from muscular tension and spasm, improved circulation and increased range of motion. I understand that massage therapists are not licensed to practice medicine and therefore do not diagnose illness or disease; perform spinal manipulations; or prescribe medical treatments. I am aware that massage therapy is not a substitute for medical treatment, medication or diagnosis for any health condition I may have. It is recommended that I pursue appropriate health management for condition I may have.

     

    THESE TERMS AND CONDITIONS CAN BE CHANGED, MODIFIED, AND/OR UPDATED BY SERENITY ON DEMAND, LLC. UPON BOOKING THE TREATMENT/SERVICE YOU AGREE TO THE TERMS AND CONDITIONS LISTED BELOW.

    • DEFINITIONS

     

    • ‘Agreed Hours’ means when the Massage Therapist will be available for the hours as agreed by the Client and SERENITY.

     

    • ‘Booking form’ means the form containing the booking details that has been emailed to and confirmed by the Client in advance of the booking.

     

    • ‘Client’ means the person and/or company for whom the services/treatment has been requested.

     

    • ‘Individual’ means the individual guest/employee of the Client attending the massage session who has requested the Service/Treatment.

     

    • ‘Massage Therapist’ means the therapist who attends the premises for the purpose of performing the service/treatment.

     

    • ‘Premises’ means the service location address as confirmed to SERENITY by the Client to perform the service/treatment.

     

    • ‘SERENITY’ means Serenity on Demand, LLC.

     

    • ‘Service or Treatment’ means the service(s)/treatment(s) requested by the Client and as shown on the Booking Form/Website.

     

    • SERVICES
      • SERENITY shall provide qualified and insured Massage Therapists to the Client at the Premises for the Agreed Hours. All therapists are licensed by the State of Florida and have a Professional Liability Insurance.

     

    • SERENITY shall instruct the Therapist to provide the service/treatment as requested by the Client.

     

    • The Massage Therapist reserves the right to provide a service/treatment for any Individual they deem until to receive such service/treatment.

     

    • All treatment provided by SERENITY will last for specified duration purchased.

     

    • SERENITY offers Party/Group Packages.

     

    • Client may purchase Gift Cards through the SERENITY website.

     

    • Gift Cards purchased are non-refundable.

     

    • Gift Cards must be redeemed within one year of purchase date.

     

     

    • CLIENT OBLIGATIONS
      • The Client must provide a suitable area for the Service to take place and a contact person for SERENITY to liaise with.

     

    • The Client is responsible for ensuring the Premises both indoors and outdoors are maintained in a safe and efficient condition.

     

    • The Client is responsible for making the Massage Therapist aware of any health and safety policies and practices relating to at the Premises and that the Massage Therapist must adhere to whilst on the Premises.

     

    • Client with pets most notify the therapist before the time of the session.

     

    • Client is responsible to inform SERENITY for any medical conditions, undiagnosed symptoms, medical treatment and medication prior to the treatment session. Failure to do so my result in incorrect treatment, for which SERENITY can accept no responsibility.

     

    • Client under the age of 18 must be accompanied by an adult. The adult must give permission for the treatment to be performed. The adult must be present throughout the treatment.

     

    • PAYMENT BY THE CLIENT
      • Payments are made online at the time of booking. The Client will be directed via the website booking form to the Payment Method (PayPal, Credit Card or Debit Card) and bookings are not confirmed until payment is made.

     

    • Clients must pay for each treatment session upfront.

     

    • Client must pay the appropriate fee for the length of their session, plus gratuity of 15%.

     

    • SERENITY shall charge the Client for any Parking Fee that the therapist

     

    • CANCELLATIONS
      • Once a booking has been placed, cancellation will incur the following charges:
        • If a cancellation is made within 30 minutes or more before a treatment, the Client may reschedule or SERENITY shall reimburse Client the full amount of the session.

     

    • Cancellations made with less than 30 minutes notice, the client will be charged $35.00.

     

    • Cancellations for Party/Group Package made less than 24 hours will incur a charge fee of 20% of the order total.

     

    • SERENITY reserves the right to cancel the provision of the Services without notice in case of the illness of a Massage Therapist or other circumstances beyond its control. SERENITY shall make reasonable attempts not to cancel the Services but the Client acknowledges that it may not be possible for an alternative Massage Therapist to attend the Premises. In this circumstance the Client will be offered an alternative day or a full refund will be issue.

     

    • SERENITY has the right to cancel a treatment without notice if the room or environment is unsuitable or inappropriate behavior, the client will be required to pay the full treatment fee and will not be offered further treatment.

     

    • PRIVACY
      • Client service/treatment details will not be discussed with anyone other than the client unless the client is under the age of 18.

     

    • Personal information will not be shared with any third parties.

     

    • SERENITY will not e-mail Client in the future unless you have given SERENITY consent.

     

    • SERENITY shall collect the following personal information:

     

    • Client name
    • Address
    • Phone Number
    • E-mail address
    • Health information/history

     

    • COMPLAINTS PROCEDURE
      • If a client wishes to make a complaint about SERENITY, it must be emailed to SERENITY at contact@serenityondemand.com.

     

    • Complaints must include the date and location of the incident, the full name of the complainant, details of complaint and a desired outcome following the complaint.

     

    • All complaints will be taken very seriously and a response will be provided with 28 days.

     

    • TERMINATION
      • This Agreement will terminate automatically if either party shall be in breach of any of its obligations under this Agreement or it either party shall have a receiver, administrative receiver, or administrator appointed, or shall enter compulsory or voluntary liquidation.

     

    • SERENITY has the right to terminate treatment/service immediately if the Client demonstrates inappropriate or sexual behavior.

     

    • LIABILITY & INSURANCE
      • The Client acknowledges that the Massage Therapist is an independent contractor and not an employee of SERENITY and that any liability as to the provision of a Treatment is between an individual and the Massage Therapist.

     

    • SERENITY is not responsible for any act or omission of the Massage Therapist or for any negligence (damages) or conduct on their part.
    • LAW
      • These terms and conditions will be governed by and construed in accordance with US law, and any disputes relating to these terms and conditions will be subject to the jurisdiction of the courts.
